Helping people define for themselves what makes a meaningful quality of life and learning how to make informed healthcare choices are set within the context of consulting with medical professionals; writing and maintaining statements of personal preferences through Advance Healthcare Directives and POLST documents; communicating preferences with family and healthcare providers; coping or caring for those with a serious illness; and exploring how to live fully in all their remaining days.
